Welcome to the Yu Dayi Medicine Clinic.
To ensure your experience is relaxing and produces optimal results,
we want to take the time to explain a few preliminary office procedures.
-
Please book online and fill out patient forms ahead of time or call our office at your convenience.
-
Each patient is required to sign an original Arbitration and Consent to Treatment form at the time of your appointment. These forms are required by our malpractice insurance carrier.
-
Our clinic is an out-of-network provider, nevertheless, we can process/submit your claim electronically. All Insurance plans have different eligibility policies for Acupuncture coverage and calling to verify is not a guarantee of coverage. We must process your claim first and await your "explanation of benefits" to properly determine your coverage. We do our best to help our patients so they can benefit from continued affordable medical care for alternative medicine.
-
Payment in full is required at the time services are rendered. We accept Visa, Mastercard, checks, and cash. (*Convenience fee of $2 for credit cards/debit cards)
-
Parking: there is free visitor parking and our office is on the ground floor. Wheel Chair accessible.
-
Treatment time: we ask that you allow adequate time for an effective treatment. Please allow yourself 1 ½ hours for the first visit, and 1 hour for each subsequent treatment.
-
Please wear loose comfortable clothing to the appointment. For Fertility & Women's Health treatments, we ask you bring a pair of shorts. (In most cases, the practitioner can administer the treatment by shifting your clothing or asking you to remove some articles. If your treatment necessitates fully undressing, we will always make you feel as comfortable as possible and provide a gown).
-
For your own safety, please maintain good personal hygiene. Before coming to your acupuncture appointment, it is important to shower - especially if you have been engaging in any physical activity or during warm weather.
-
We recommend a small healthy snack approximately 1 hour before your treatment. Skipping meals before your treatment may cause you to feel dizzy or light-headed. Following your treatment, you are advised to drink room temperature or warm water.
